Job Description
HCA Healthcare is seeking a Home Health Clinical Liaison to coordinate smooth transitions of care from hospital to home health services. You will assess patient needs, educate patients and families on home health options, and collaborate closely with physicians, case managers, and home health teams to develop safe discharge plans. The Liaison will review clinical documentation, ensure eligibility and regulatory compliance, and communicate benefits and services clearly. This role supports patient-centered care, drives appropriate referrals, and helps optimize outcomes, satisfaction, and continuity of care in a fast-paced healthcare environment. Responsibilities Serve as primary liaison between hospital, physicians, case management, and home health agency Conduct clinical assessments to determine home health eligibility and appropriate level of care Educate patients and families on home health services, benefits, and care plans Coordinate and facilitate smooth transitions from inpatient or outpatient settings to home care Review and interpret clinical documentation to ensure regulatory and payer compliance Initiate and process home health referrals and authorizations accurately and timely Communicate effectively with interdisciplinary teams to optimize patient outcomes Track, document, and report referral activity and patient status in EMR systems Support quality, patient satisfaction, and readmission reduction initiatives Represent HCA Healthcare home health services to internal and external stakeholders Required Skills Clinical assessment Discharge planning Home health regulations Care coordination Patient and family education Electronic medical records (EMR) Interdisciplinary communication Utilization management Time management Data and documentation accuracy
