The ideal candidate will possess excellent communication skills, a strong understanding of healthcare support processes, and the ability to manage multiple outreach initiatives effectively. As a Patient Outreach Coordinator, you will be responsible for engaging with patients, providing information about healthcare services, and ensuring seamless communication to enhance patient satisfaction and health outcomes. Duties Contact patients via phone, email, or other communication channels to provide appointment reminders, follow-up care instructions, and health education. Manage incoming calls efficiently, addressing patient inquiries with professionalism and empathy while maintaining excellent phone etiquette. Maintain accurate electronic health records (EHR) and data entry logs to ensure up-to-date patient information. Assist patients with understanding health insurance details and navigating healthcare support services. Collaborate with healthcare teams to coordinate patient appointments, referrals, and necessary documentation. Utilize Microsoft Office tools to generate reports, update records, and communicate effectively within the team. Support the implementation of outreach campaigns aimed at improving patient engagement and compliance. Demonstrate technical proficiency in EMR/EHR systems and electronic health records management for accurate documentation. Skills Multilingual abilities are highly desirable to serve diverse patient populations effectively. Strong computer skills with proficiency in Microsoft Office applications and healthcare software such as EMR/EHR systems. Excellent phone call management skills combined with effective verbal communication in English; bilingual capabilities are a plus. Knowledge of medical terminology and health insurance processes to facilitate clear communication with patients. Experience in customer service or client services within a healthcare setting, emphasizing problem-solving skills. Familiarity with call center operations and phone etiquette to handle high-volume interactions professionally. Ability to perform data entry accurately while managing multiple tasks efficiently under pressure. Demonstrated interpersonal skills that foster trust, empathy, and positive engagement with patients from diverse backgrounds.
