The Specialty Care Clinic of the Clarke County Health Department is seeking a Social Worker to provide culturally appropriate psychosocial assessments to individuals living with
HIV/AIDS.
The individual must be familiar with providing trauma-informed care and navigating the health care system. Responsibilities include ensuring clients are eligible for Ryan White services, creating individual service plans using a strengths-based approach, crisis intervention, connecting clients with appropriate community resources, and assisting with medication assistance programs. The individual will facilitate whole-person care by working with an interdisciplinary team to create meaningful interventions with the goal of the client becoming undetectable. As a Case Manager within the Specialty Care Clinic at the Georgia Department of Public Health, you will join a dedicated group of public health professionals that help support the overall mission and vision of protecting lives of Georgia residents in a variety of ways. Job Responsibilities Ensure each client is eligible for Ryan White Services. Conduct psychosocial assessments. Provide referrals to appropriate community services. Advocate for clients and assist with removing barriers to care. Prepare and update service plans as outlined in the Ryan White Case Management Standard Operating Procedures. Collaborate with medical staff to develop effective treatment plans to enhance health outcomes. Maintain knowledge of current best practices and implement interventions based on empirical evidence. Adhere to the NASW Code of Ethics and clinic protocols. Provide timely and thorough documentation of interventions, phone calls, and services provided. Provide crisis intervention services and referral to appropriate care. Assist clients with applying for medication assistance programs. Minimum Qualifications Master's degree in social work and counseling from an accredited college or university and eligibility to be licensed.
