Job Description
CenterWell Home Health is seeking a Patient Service Coordinator-LPN to support in-home care for seniors and adults with chronic conditions. In this role, you'll coordinate patient services, perform clinical tasks within LPN scope, and support care planning to promote safety and independence at home.
