This role provides operational oversight, strategic direction, and leadership to ensure coordinated, patient-centered support across the continuum of care. Responsibilities include improving patient outcomes, reducing unnecessary utilization, optimizing healthcare resources, and partnering with clinical, operational, and external stakeholders to drive high-quality care delivery. Responsibilities Provide strategic leadership and operational oversight for Integrated Care Management (ICM) programs across hospital and ambulatory settings. Lead multidisciplinary teams including RN Care Managers, RN Care Coordinators, and Licensed Social Workers to deliver high-quality, patient-centered care. Build strong partnerships with clinical leaders, community organizations, and cross-functional stakeholders to support seamless care integration. Drive process improvement initiatives focused on reducing readmissions, improving patient flow, optimizing length of stay, and enhancing care coordination outcomes. Ensure compliance with CMS, DNV, OSHA, OIG, and other federal and state regulatory requirements. Coach, mentor, and develop team members while fostering a culture of collaboration, accountability, and service excellence. Qualifications Education:
