Job Description

St. Luke's Lifestyle Medicine uses medically supervised evidence-based lifestyle interventions like supervised exercise, education, counseling and health coaching to help patients make therapeutic lifestyle changes to treat chronic conditions like cardiovascular disease, pulmonary disease, type 2 diabetes and obesity. The care is organized around the 6 pillars of lifestyle (nutrition, exercise, sleep, risky substance use, stress management, and positive social connection). The treatment team consists of members from these disciplines: physician, NP/PA, RN, RD, RT, PT, OT, EP, LCSW and pharmD.
What You Can Expect:
Ensures that patient needs are met through effective management of multiple sites or disciplines. Implement effective operational processes and lead process improvement work groups. Assumes responsibility for the efficient and profitable management of rehabilitation services in assigned area(s)/setting(s) of service. Develop policies, procedures, service standards, and work flows to support the consistent application of practices across function. Provide leadership, guidance, counseling and continuing education to staff. Select, develop, coach, mentor, and assess performance of staff. Scope of education and performance assessment may include other leaders. Design, develop and implement strategies for continued growth of the rehabilitation organization. Leads the budget and goal setting process. Administers interventions consistent with treatment plan based on evidence based best practice, provider protocol and governing standards. In conjunction with clinical program leaders and Medical Director of the Service, monitors the standard of clinical services being delivered across setting and ensures an ethical and compliant product is being delivered.

Education:
Bachelors degree
Experience:
4 years relevant experience
Licenses/Certifications:
Valid license to practice in one of the following: Nursing, Respiratory Therapy, Exercise Physiology, Physical Therapy, Occupational Therapy AND Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross.
Preferred Qualifications:
Prior Cardiopulmonary Rehab Experience
