Job Description
Therapy Manager - Home Health Full-Time | Leadership | Growing Agency Astra Health is continuing to grow and is seeking an experienced Therapy Manager to lead our home health therapy team. This is a key leadership role responsible for overseeing PT, OT, and SLP services , driving clinical excellence, and ensuring strong patient outcomes in the home setting. Why Join Astra Health? Growing, stable home health agency Strong clinical and leadership support Opportunity to build, lead, and shape the therapy department Autonomy with support—your voice matters Meaningful work impacting patients in their homes Key Responsibilities Lead, mentor, and supervise a team of Physical, Occupational, and Speech Therapists Oversee daily therapy operations , including caseload distribution and scheduling Ensure compliance with Medicare, OASIS, and home health regulations Review and guide plans of care, documentation, and clinical quality Monitor patient outcomes, productivity, and quality metrics (SHP, timeliness, etc.) Collaborate with nursing, intake, and physicians to ensure coordinated care Support Start of Care (SOC) workflows and therapy utilization Manage staff performance, coaching, and annual evaluations Lead recruitment, onboarding, and training of therapy staff Drive process improvement initiatives to enhance efficiency and patient outcomes Act as the clinical resource for therapy-related questions and escalations Qualifications Licensed PT, OT, or SLP - Illinois Home health experience Prior leadership or supervisory experience strongly preferred Strong understanding of Medicare guidelines, OASIS, and home health operations Proven ability to manage teams, prioritize, and drive results Excellent communication and leadership skills Compensation & Benefits Competitive salary (based on experience) Medical, Dental, Vision, 401(k), Aflac PTO + Sick Time Mileage reimbursement Apply Today Join a team where your leadership makes a difference.
