Job Description
Overview Join our dynamic healthcare team as a Clinical Case Manager, where you will play a vital role in coordinating patient care at Blooming Horizons Center. This energetic position offers the opportunity to utilize your clinical expertise to improve patient outcomes! We are seeking a passionate professional committed to delivering exceptional patient service and advancing healthcare delivery through comprehensive case management, while managing the utilization hours of the clients to maximize the child's learning through their ABA prgrams written by the BCBA's. Responsibilities Conduct thorough patient assessments, including medical history review, physiological evaluation, and diagnostic evaluation to determine individual care needs. Coordinate services, ensuring timely access to appropriate resources such as behavioral health, mental health counseling, ABA Therapy, Speech Therapy and social work support. Oversee intake procedures and coordinate diagnostic evaluations to ensure accurate patient assessment and appropriate treatment planning. Maintain detailed documentation of case management activities within medical records systems while adhering to privacy standards and hospital policies. Skills Extensive Clinical experience, particularly within clinic or center setting. Proven expertise in managing complex cases involving behavioral health, mental health counseling, pediatrics. Experience with inpatient/outpatient clinic operations. Skilled in utilization management, patient service coordination, patient care delivery, and diagnostic evaluation techniques. Familiarity with managed care principles and medical records management systems. Excellent communication skills for effective patient assessment and interdisciplinary collaboration while maintaining HIPAA compliance.
