Care Manager (RN) Full-Time| Louisville, KY & Surrounding Areas Diversified Life Care Partners (DLCP) is growing and seeking an experienced, compassionate Registered Nurse Care Manager to join our Aging Life Care™ team. What You'll Do As a Care Manager, you'll serve as a trusted advocate and advisor, helping clients maintain independence, safety, and quality of life.
Responsibilities Include:
Conducting comprehensive in-home assessments to identify client needs, risks, and goals Developing individualized care plans that address medical, social, financial, and personal well-being Advocating for clients across healthcare, community, and home settings Coordinating services such as home care, rehabilitation, transportation, social enrichment programs, and specialty providers Supporting clients and families through crises, hospitalizations, transitions, and difficult decisions Collaborating with physicians, therapists, attorneys, financial professionals, and community partners Documenting assessments, recommendations, interventions, and outcomes in a secure electronic platform Monitoring client progress and adjusting care plans as needs evolveWhat We're Looking For Required Qualifications:
Active Registered Nurse (RN) license Minimum of 5 years of relevant clinical experience, preferably in: Geriatrics Home Health Hospice Case Management Care management Strong knowledge of aging-related issues, chronic disease management, and community resources for older adults Exceptional communication, relationship building, and problem solving skills Ability to work independently while managing multiple clients and priorities Proficiency with email, electronic documentation systems, and mobile scheduling tools Reliable transportation and willingness to travel throughout Louisville, Lexington, and Southern Indiana Why Join DLCP Meaningful Work- Make a real difference in the lives of older adults and their families.
