A medical facility located near Shelbyville, KY is seeking a full-time Director of Case Management to join its team. Director of Case Management ready to join a community of individuals passionate about what they do need apply! Details are below.
Pay:
$100,000 - $120,000/
Year Director of Case Management Opportunity:
Full-time permanent position Director of Case Management manages the daily operations and HR functions of the Case Management department Directs the interdisciplinary care, discharge planning, and patient outcomes while championing patient advocacy Guides and trains staff, builds relationships with healthcare and insurance networks, and fosters a positive, celebratory work culture
Director of Case Management Qualifications:
Must be a licensed clinician (ex: RN, SW, OT, PT, ST, Rehabilitation counseling, etc.) Current CCM or ACM certification or eligibility to obtain within first year of employment Must possess a bachelor's degree; graduate degree is preferred Three years of hospital-based Case Management experience, including Utilization Review and Discharge Planning experience and leadership experience
