Job Description:
SCOPE:
The Survivorship Coordinator is a clinically active role responsible for the hands-on implementation and day-to-day operations of the cancer survivorship program at Mary Bird Perkins Cancer Center. This individual will work directly with patients and clinical teams to deliver survivorship care—preparing charts for visits, supporting or conducting patient encounters, coordinating follow-up, and ensuring care plans are completed and communicated. This role is best suited for a highly motivated Medical Assistant (MA) or Licensed Practical Nurse (LPN) with strong clinical instincts, a patient-first mindset, and the organizational skills to keep the program running smoothly.

1. Clinical Support and Chart Preparation 2. Survivorship Care Plan Completion and Coordination 3. Coordination of support groups and survivorship events 4. Institutional Engagement and Accreditation Support 5. Other duties as assigned
QULAIFICATIONS
High school diploma or GED required. Completion of an accredited Medical Assistant program or LPN licensure required. Active LPN licensure in Louisiana preferred. Minimum 2 years of clinical experience as an MA or LPN, preferably in oncology, primary care, or chronic disease management. Experience with chart preparation, patient intake, and EMR documentation required. Prior experience in a survivorship, care coordination, or navigation setting strongly preferred. Familiarity with oncology workflows and multidisciplinary care teams a plus.
