Job Description

Compass Health, a leading provider of psychiatric services, is currently accepting resumes for aCareConnect Manager at our Home Office in Crowley office. We are looking for professionals who share our commitment to improving lives by passionately serving others. Job Summary We are seeking a dedicated and dynamic CareConnect Manager to oversee our CareConnect Department and Coordinators. The CareConnect Manager is an effective communicator and manager of information in coordinating health care decisions. Participates in huddles with Care Navigators and Inpatient Facilities as needed. Reviews referrals and coordinates placement. Reports on MC conversion rates and updates referrals to Avatar tracking report. Your leadership will be instrumental in enhancing patient outcomes and optimizing care delivery across multiple settings. Duties Lead and supervise a multidisciplinary team of care coordinators and healthcare professionals to deliver coordinated patient care. Manage data collection efforts related to patient outcomes, utilization review, and compliance documentation review. Maintains ongoing relationship with the medical community for purposes of determining and meeting psychiatric health care needs. Ensure adherence to healthcare regulations such as Medicare requirements while maintaining high standards for documentation accuracy and medical records management. Promote a culture of continuous improvement by providing leadership in behavioral health initiatives, addiction counseling programs, and managed care strategies. Demonstrated supervisory skills with experience managing teams within complex healthcare systems. Familiarity with HIPAA regulations, medical terminology, insurance verification, and medical records documentation review. Leadership qualities that foster teamwork, accountability, and professional development among staff members.
