Job Description
Element Care is seeking a Project Manager, Clinical & Operational Innovations to lead initiatives that enhance care for frail seniors in our PACE program. In this role, you will plan and manage cross-functional projects that improve clinical quality, streamline operations, and support seniors living safely in their communities. You'll collaborate with interdisciplinary teams, analyze workflows and data, design and implement new care models, and track outcomes. This position offers the opportunity to drive meaningful change in a compassionate, patient-centered, community-based environment.
