This position supports effective care coordination and continuity of care for patients. The PSC also assists with day-to-day office operations and staff coordination under the direction of the Clinical Operations Manager. Key Responsibilities Manage and maintain schedules for assigned patients; adjust schedules promptly when visits require reassignment Answer incoming calls regarding patient and clinician schedules; resolve routing issues and escalate clinical concerns as needed Prepare and review weekly and weekend clinician schedules for accuracy Collaborate with Clinical Director and Clinical Care Manager to ensure clinician productivity targets are met Schedule all Admissions, Recertifications, and Resumption of Care visits Reschedule missed or declined visits when appropriate Ensure sufficient clinician coverage for vacations, sick leave, and holidays Maintain accurate tracking of patient data and clinician routes Run daily reports and address discrepancies or scheduling issues Assign appropriate visit reasons based on insurance authorizations and clinical documentation Ensure compliance with prescribed visit frequency for all assigned patients Support internal patient transfers between branch offices Follow up on scheduling or coordination issues identified during after-hours operations Coordinate training visits and onboarding schedules for new clinicians Monitor clinician utilization and report gaps to leadership Track and report clinician and patient metrics as required Provide coverage for other PSCs as needed Maintain compliance with HIPAA, privacy standards, and company policies Perform additional duties as assigned Required Qualifications Bachelor's degree or equivalent combination of education and experience Strong proficiency with Microsoft Office (Outlook, Excel, Word, Teams) Preferred Qualifications Degree concentration in Social Work, Human Services, Psychology, or Business 1+ year of experience in a home health or healthcare environment
