30 am - 4 pm, Full ScheduleSUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
- Family Services
- Responsible for the provision and/or monitoring of person centered planning to an assigned caseload.
This includes:
identifying and assessing dreams/desires/needs during the person centered plan and/or goal provision process, assisting with managing guardianships and Medicaid/entitlements, coordination of Vocational Profile/Employment Plan with input from Skills Trainers and Employment Representatives, development of individualized person centered plan of service, linking and coordinating with identified supports, monitoring, reviewing and amending of supports and services at intervals identified in the plan, and maximizing the participation and independence of individuals receiving support services.- Also responsible for the completion of monthly Supports Coordination contacts and clinical contact notes, completion of regular review of all goals as outlined in the person centered plan and/or goal provision, and clinical case record maintenance including ensuring compliance with consents and physicals.
CLIENT'S REQUIRED SKILLS & EXPERIENCE
- A minimum of three (3) years of professional exerience in mental health, developmental, or disabilikty services.
- Proficiency in Microsoft Office required.
- Required LMSW or LPC
- Valid Michigan Driver License
