- In Office role Position Summary The Home Health Clinical Supervisor is responsible for the daily clinical operations of the home health field staff.
Key Responsibilities Clinical Oversight & Case Management Care Coordination:
Review and approve patient intake data, plans of care, and clinical schedules to optimize service delivery.OASIS Integrity:
Audit Outcome and Assessment Information Set (OASIS) documentation for accuracy, clinical consistency, and timely submission.Case Conferences:
Lead regular multidisciplinary case conferences to discuss patient progress, discharge planning, and utilization review.Clinical Consultation:
Provide real-time clinical guidance, decision support, and problem-solving assistance to field clinicians in the community.Personnel Management & Supervision Performance Evaluations:
Conduct joint field visits to assess clinician competency, provide direct feedback, and complete annual evaluations.Staff Development:
Onboard, mentor, and train new clinical staff on agency policies, documentation standards, and clinical protocols.Resource Allocation:
Manage staff caseloads and geographical assignments to maximize productivity and prevent clinician burnout.Regulatory Compliance & Quality Assurance Survey Readiness:
Maintain continuous compliance with Medicare Conditions of Participation (CoPs), state licensing regulations, and accrediting bodies (e.g., Joint Commission, CHAP).QAPI Participation:
Active member of the Quality Assessment and Performance Improvement (QAPI) committee, assisting with chart audits and corrective action plans.Incident Management:
Investigate and document patient complaints, adverse events, and clinical variances, implementing systemic fixes when needed.Qualifications & Skills Education & Licensure Professional Credentials:
Active, unencumbered license as a Registered Nurse (RN) or Physical Therapist (PT) in the state of practice.Academic Background:
Bachelor's degree (BSN orBSPT/DPT
) preferred.Experience Home Health Experience:
Minimum of 3 years of hands-on clinical experience in a Medicare-certified home health agency.Leadership Background:
At least 1 year of supervisory, charge nurse, or clinical lead experience preferred.Core Competencies Regulatory Mastery:
Deep, working knowledge of Medicare home health regulations, OASIS-E guidelines, and PDGM reimbursement models.Technical Skills:
High proficiency with Electronic Medical Record (EMR) software (e.g. Kinnser/WellSky).Communication:
Exceptional interpersonal skills to manage staff conflicts, interface with physicians, and address patient families.Critical Thinking:
Strong clinical judgment with the ability to manage multiple operational priorities simultaneously.Pay:
$80,000.00- $85,000.
