$21.31 - 28.84 per hour How you'll make an impact in this role: Coordinate participant appointments and services, including care provided outside of the PACE center. Work with external vendors, providers, and community partners to schedule and coordinate participant services. Communicate with participants, families, and the care team to keep appointments and services on track. Help ensure participants have access to the care, transportation, and support services they need. What minimum qualifications you'll need:

Education:

High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
