Full Job Description Job Summary:
Affinity Care MN is seeking a mission-driven Manager of Case Management (MCM) to lead a team of Case Managers. The MCM will provide strategic leadership, enhance employee retention, and foster strong relationships with internal and external stakeholders while ensuring high-quality, compliant service delivery. This role requires community engagement and occasional travel to meet with staff.
Job Responsibilities:
Oversee Waiver Case Managers working with program participants, setting performance standards, and tracking progress toward goals. Coach and develop Waiver Case Managers to meet goals and benchmarks. Develop and implement strategies to improve team productivity, efficiency, and service delivery. Create and maintain retention and engagement systems to address team concerns and support staff growth. Serve as a subject matter expert, ensuring high-quality, compliant services. Hire, review, and terminate direct reports according to organizational policies. Plan, coordinate, and lead team meetings with attention to engagement and collaboration. Ensure accurate documentation and timely billing for services, maintaining compliance with best practices. Manage budgets and expenses, ensuring adherence to staffing and spending expectations.
Job Qualifications:
Must have the following Bachelor's Degree in Social Work, Human Services, Psychology, or a related field. Minimum of two years of supervisory or leadership experience. Minimum of two years of experience in waivered case management. Proficiency with Microsoft Office, Google Workspace, and care-related software platforms. Must also be able to meet the following qualifications: Valid driver's license Must not be disqualified by appropriate criminal background studies. Complete all orientation and training needs as written in the policy. Possess good oral and written communication skills and organizational skills.
