Brooklyn Park, MN TYPE Direct Hire ID j-19750 POSTED Jun 8, 2026 Seeking top talent, dedicated full time Home Health RN Patient Care Managers for a national leader in personal care, medical and non medical home health, hospice, and palliative care services needed in 2 areas of Minnesota including (1) in Burnsville and (1) in Brooklyn Park working Mon-Fri 8a-5p! The right individuals will be responsible for the direct management and leadership of the clinical teams! Must be a current RN licensed in the state of MN with previous experience in home care setting with 2 years management or supervisory experience preferred.
