JOB DESCRIPTION ABOUT THE JOB
The RN Care Manager provides patient-centered clinical care management for hospitalized patients in inpatient and emergency department settings. Serving as an integral member of the interdisciplinary healthcare team, this role conducts comprehensive assessments, develops and implements care plans, and coordinates interventions to support safe, timely, and effective transitions of care. The RN Care Manager identifies barriers to progression, facilitates discharge planning, and applies evidence-based practices to promote quality outcomes and the clinically appropriate, cost-effective utilization of healthcare resources. This is a full-time (40 hours/week) position.Responsibilities:
Identifies and prioritizes high-risk patients to support timely, effective care transitions. Conducts comprehensive, patient centered assessments to identify gaps in care and implement appropriate interventions, education, referrals, and consultative services Provides clinical care coordination to diverse patient populations, utilizing evidence-based practices to develop and adjust transition plans based on clinical needs, available resources, and patient preferences, including cultural considerations. Actively monitors and revises transition plans in response to evolving patient conditions and care needs. Provides timely progression of care through proactive identification, communication, documentation, and resolution of barriers or delays. Coordinates transitions to the appropriate level of care (e.g., home, home care, SNF/TCU, hospice, community-based services), serving as a liaison between patients/families, the interdisciplinary team, and external agencies to ensure high-quality, cost-effective outcomes. Applies knowledge of community resources, insurance coverage, healthcare funding, and applicable county, state, and federal regulations when planning transitions of care. Provides education to patients and families regarding health conditions, care plans, and available community programs and resources. Documents care management activities accurately, clearly, and timely in the electronic medical record using standardized formats. Maintains confidentiality in accordance with organizational policies, HIPAA, state statutes, and applicable regulatory requirements. Knowledge of county, state, and federal regulations governing healthcare funding, mandated reporting obligations (including child abuse and vulnerable adult reporting), and regulatory compliance. Establishes and maintains positive, collaborative working relationships with interdisciplinary team members, patient care units, and community partners Collaborates with and provides informal education to the interdisciplinary team regarding patient care discussions and transition planning Functions both independently and as an effective team member of the interdisciplinary team to support high-quality, efficient patient care Other duties as assignedABOUT YOU
Associate's or Bachelor's Degree in Nursing Licensed Registered Nurse (RN) - MN Board of Nursing Minimum of 2 years hospital/healthcare experience 2 years experience in care management related role (preferred) Case Management Certification (preferred)ABOUT OLIVIA HOSPITAL & CLINIC
