This is not a traditional patient scheduling position. You'll be helping manage the front end of the home health admission process , making sure referrals are complete, eligibility is verified, documentation is collected, information is entered accurately, and our clinical team has what they need to begin care. Verify insurance coverage, payer information, and reimbursement eligibility . Review physician orders and supporting medical documentation. Contact hospitals, clinics, physician offices, and referral sources for missing information. Communicate with patients and families regarding the intake process. Enter accurate and timely information into the electronic medical record. Track referrals from initial receipt through the admission process. Communicate with nursing, therapy, intake, and operational teams. Identify missing information that could delay admission and proactively work to resolve it .
