Neither Does Great Coordination. By Friday afternoon, hospitals may be preparing patients for discharge. Physician orders are still coming through. Families still have questions. New referrals still need to be reviewed. And patients still need care. At Home Health Care, Inc., we're looking for an experienced healthcare professional who can help keep that process moving throughout the weekend. If you've worked as a Health Unit Coordinator, Unit Coordinator, Medical Unit Coordinator, Intake Specialist, Admissions Coordinator, Referral Coordinator, Health Unit Clerk, or Medical Office Coordinator , your experience could translate extremely well to this position. This is not a traditional patient scheduling position. You'll be helping manage the front end of the home health admission process , making sure referrals are complete, eligibility is verified, documentation is collected, information is entered accurately, and our clinical team has what they need to begin care. Some of the strongest intake professionals don't necessarily start their careers in home health. They come from hospital units, clinics, physician offices, admissions departments, and other healthcare environments where they've learned how to keep complicated processes organized . You already understand what happens when documentation is missing. You know medical terminology. You're comfortable communicating with nurses and providers. You know how to prioritize when five things need your attention at once. And when something isn't right, you know how to track down the answer instead of letting it sit . Those are exactly the skills we're looking for. What You'll Be Doing Review incoming home health referrals for completeness and admission eligibility. Verify insurance coverage, payer information, and reimbursement eligibility . Review physician orders and supporting medical documentation. Contact hospitals, clinics, physician offices, and referral sources for missing information. Communicate with patients and families regarding the intake process. Enter accurate and timely information into the electronic medical record. Track referrals from initial receipt through the admission process. Communicate with nursing, therapy, intake, and operational teams. Identify missing information that could delay admission and proactively work to resolve it . Prioritize multiple referrals while maintaining accuracy and excellent service. Backgrounds That Translate Well We'd especially like to hear from: Health Unit Coordinators (HUC). Unit Coordinators. Medical Office Coordinators. Patient Administration Specialists. Healthcare Administrative Professionals. Previous home health intake or admissions experience is strongly preferred , but we're also interested in experienced healthcare professionals whose skills translate directly into intake. What We're Looking For Previous healthcare office experience. Strong knowledge of medical terminology and healthcare workflows . Excellent written and verbal communication skills. Strong computer and EMR abilities. Exceptional organization and attention to detail. Ability to manage competing priorities without losing accuracy. Confidence communicating with clinical professionals and referral sources. Strong follow-through and problem-solving abilities. Experience with home health admissions, referral processing, insurance verification, payer guidelines, Medicare requirements, or healthcare intake is highly preferred. Own the Weekend.
