Job Description

Company Overview Provide Care, Inc. is dedicated to delivering stability and high-quality services to individuals in our care while fostering growth and independence within the community. We operate 18 Adult Foster Homes across Isanti, Pine, and Chisago counties, committed to enhancing the quality of life for those we serve. Summary As a House Manager at Provide Care, Inc., you will play a vital role in overseeing the daily operations at one of our High Medical Adult Foster Homes. Located in Wyoming, MN, this position is a full-time evening, this is not a daytime position. House Managers are to maintain a safe and positive environment for each individual to reach their maximum potential. Salary is negotiable, this particular house is a high medical needs house with total cares. Responsibilities 1. Schedule staff in home to meet the individual's needs. 2. Ensure staff has the necessary skills and tools to meet the individual's needs. 3. Maintain a safe, healthy, positive environment for the individuals we support. 4. Schedule necessary medical appointments for the individuals. 5. Ensure staff is recording data for each individual we support. 6. Coordinate with the Pharmacy for individual's medications or Medical Supply Companies. 7. Initial and ongoing contact with families, county Case Managers, and Extended Support Team Members. (ie.
CSS, ARMHS
etc.) 8. Schedule and conduct monthly staff meetings. 9. Complete required reports; Quarterly, Semi-Annual, Annual, financial, incident/accident, etc. 10. Orientation of new staff to site specific training. 11. Assist individual's as necessary in money management. Keep "All" receipts and record "All" transactions to ensure accurate record keeping of individual's funds. 14. Keep the Designated Coordinator, Program Director and Director of Program Services informed. 15. Perform all duties of DSP while working with individuals we support. 16.Other reasonable duties as assigned. Requirements Minimum of High School diploma or GED. Experience in home community care or group home settings with high medical needs. Strong understanding of working with individuals with developmental disabilities. Ability to lift heavy objects as needed for resident support. Excellent communication skills for effective interaction with residents, staff and teams of the individuals. Experience in medication administration. Two-year or four-year college degree plus 2000 hours of documented Mental Health Clinical Supervision. Without a degree, 6,000 hours of documented Mental Health Clinical Supervision.
