Job Overview Ebenezer is seeking a Licensed Practical Nurse (LPN) for our Skilled Nursing Community in Wyoming, Minnesota! This TCU nursing provides for and assists in meeting the residents' physical, emotional and psycho-social needs while adhering to all facility, state and federal guidelines, Ebenezer Standards and Policies This Nursing schedule includes; 48 hours every two weeks Day/evening shift; 12 hour shift Every other weekend and holiday rotation Meadows on Fairview TCU is located in the city of Wyoming, MN, steps away from the Fairview Lakes Hospital and a short distance from Forest Lake and North Branch. We offer a home like atmosphere where you feel like you belong the minute you arrive! Our 14-bed Transitional Care Unit is small enough to get to know your co-workers and residents yet large enough to provide you with opportunities and room to grow.

Job Duties:

Monitors personal cares provided to assure consistency with each individual plan of care Administers medications and performs treatments per physician orders and facility policies Assures that medical records and other documentation are current, complete, and accurate. Provides data relevant to the development of the plan of care Performs other duties as assigned and directed

Required:

MN LPN licensure AHA BLS for Healthcare Providers or Red Cross CPR for Professional Rescuers BLS is required upon hire and/or new hire must show proof that they are currently enrolled in a BLS class to renew or obtain their certification. BLS certification must be obtained within three-weeks of hire date and before nurse can perform independent practice 0-1 years of experience Ability to use technology in the form of computers, hand held devices, cell phones, and fax machines

Preferred:
