Job Description

Job Description Summary :
The Healthcare Home Director oversees the implementation and coordination of Healthcare Home. They provide services to individuals impacted by a variety of behavioral health (addiction/substance abuse and mental health) issues, recognizing that the individuals we serve often times have experienced trauma that affects their development and adjustment. They demonstrate a warm and welcoming empathic, hopeful attitude, conveying a philosophy of dual recovery. They will be trained in trauma-informed care and will work to help provide services in an environment that is sensitive and responsive which will prevent victimization, abuse, or trauma as a result of the care received. They report to the Director of Adult Community Services.
Education:
Must possess a current Missouri license as a Registered Nurse and maintain current licensure throughout employment. Requires valid driver's license.
Responsibilities:
Effectively trains staff on health information training (HIT) tools and initiatives and provides technical assistance when needed. Effectively manages HCH team to ensure required services and documentation are completed fully and timely. Ensures Nurse Care Manager's (NCM's) adequately manage their caseloads assigned by HCH Director. Ensures NCM's attend weekly team meetings assigned and participate actively. Ensures effective coordination of hospital admissions/discharge planning, including medication reconciliation and hospital alerts are promptly acted upon by the HCH team. Conducts scheduled and ongoing quality assurance HCH activities to ensure services and documentation are completed per standards. Annually, and as needed, completes HCH policy manual reviews and revisions to ensure program is in compliance with CARF, DMH, CCBHO and other entities involved with standards of care within timeline given. Ensures that HCH reporting measures are completed and submitted in a timely manner to DMH, including the HCH Monthly Implementation Report. Ensures compliance with required DMH expectations and timelines for HCH enrollment, dis-enrollment and transfer notifications. Proactively educates program staff and other departments and community members about Healthcare Home and chronic health diseases. Perform all other duties as assigned.
Physical Requirements:
Normal Office environment. Typical physical demands include prolonged sitting, frequent standing, bending, stooping, and occasional lifting. Requires normal range of hearing and manual dexterity sufficient to operate keyboard, telephone, photocopier, calculator, and other office equipment as needed. Requires ability to work under stressful conditions and to work irregular hours and deal with irate individuals. Requires use of a computer TB test upon hire.
