• Full-time Overview We are seeking an energetic and dedicated Health & Wellness Director to lead our comprehensive health services within an assisted living and memory care community.

In this pivotal role, you will oversee the development, implementation, and management of resident care programs, ensuring the delivery of high-quality, person-centered health services. Your leadership will foster a positive environment that promotes wellness, safety, and independence for our residents while maintaining compliance with all regulatory standards. Job Duties and Responsibilities Oversee all clinical operations to ensure high‑quality resident care in compliance with Missouri DHSS regulations Complete and maintain resident assessments, service plans, and level‑of‑care evaluations Coordinate care with physicians, hospice, home health, therapy providers, and other healthcare partners Supervise, train, and evaluate Health and Wellness staff and ensure required competencies are completed Ensure compliance with medication management procedures and documentation standards Monitor resident needs, changes in condition, and protective oversight requirements Conduct quality assurance activities, internal audits, and self‑surveys to maintain regulatory readiness Manage incident reporting, investigations, corrective actions, and communication with DHSS as required Ensure infection control practices, safety procedures, and sanitation standards are followed Participate in resident selection, move‑in assessments, and transition planning when higher care levels are needed Maintain accurate clinical documentation, training records, and regulatory compliance files Support a positive environment for residents, families, and staff through effective communication and leadership. Minimum Qualifications Active, unencumbered LPN license in the State of Missouri Graduate of an accredited School of Nursing Community‑Based Assessment (CBA) certification or ability to obtain within 90 days Minimum of 2 years of experience in Assisted Living and/or Memory Care Current CPR and First Aid certification or ability to obtain upon hire No findings of abuse, neglect, or mistreatment in a healthcare setting Freedom from illegal drug use and effects of alcohol in the workplace Knowledge of geriatric nursing principles and Missouri

regulations Ability to complete assessments, service plans, and change‑of‑condition evaluations Ability to supervise, train, and evaluate Health and Wellness staff Strong written and verbal communication skills Ability to make independent decisions, analyze problems, and implement solutions Ability to pass background checks and Family Care Safety Registry screening Preferred Qualifications Three years of management or leadership experience in senior living, LTC, or home health Experience working in Memory Care, including behavioral documentation and interventions Experience conducting staff competencies and training (L1MA, CMT, CNA, Care Partners) Familiarity with infection control practices and regulatory reporting requirements Experience with electronic health records and clinical documentation systems Geriatric nursing certification Experience participating in QAPI, audits, or internal compliance reviews Experience coordinating with hospice, home health, therapy, and other healthcare partners Join us as our Health & Wellness Director to lead compassionate care initiatives that enhance residents' quality of life.
