Shodair is looking for a Float Patient Care Coordinator to join the team! The Float PCC does not have an assigned patient caseload; however, the position supports Acute, Residential and Resource Patient Care Coordinators with coverage needs. The Float Patient Care Coordinator provides skills coaching to patients in a group setting at least 4 times weekly and participates in 1:1 care coordination service.

What You'll Do:

The Float PCC does not have an assigned patient caseload; however, the position supports Acute, Residential and Resource Patient Care Coordinators with coverage needs. Provides skills coaching to patients in a group setting at least 4 times weekly and participates in 1:1 care coordination service. Coordinates care to patients by developing, monitoring, and coordinating interdisciplinary care in conjunction with the treatment team. Provides effective leadership and advocacy in building relationships between teams, families, and other community professionals. Completes their own billing and note documentation to comply with hospital and governing agency standards. Assists families in navigating community services and systems and can help in the application and referral process. This position starts at $23.65 per hour!

Requirements:

EDUCATION, TRAINING AND EXPERIENCE

Post-secondary education or Bachelor's degree in a human services field strongly preferred. At least 3 year of experience serving people with mental illness is required. 5 years of experience in case management or patient care in a healthcare or social services delivery system preferred. 1 Year experience in a patient care setting.
