Integrated Care for Greater Health Integrated Care for Greater Health is a leader in providing comprehensive, evidence-based mental health and substance use disorder treatment services. We are looking for a passionate and dedicated Clinician to join our team.
Position Overview:
As a Clinician, you will coordinate and monitor the array of services and supports identified in each service user's person-centered plan. These interventions are strength-based and focus on the promotion of recovery, symptom reduction, increased coping skills, and the achievement of the highest level of social functioning.
Key Responsibilities:
Conduct outpatient therapy including individual, family, group, SAIOP and SACOT sessions. Provides group leadership to include education and skills-related groups Manages and monitors caseload of active treatment for persons served Perform ongoing screening for mental health/substance abuse or other issues including use of initial and ongoing psychometric testing Assisting with intake process including triage and review of intake documentation Orient new persons served to ICGH Conduct Comprehensive Clinical Assessments Develops person-centered profiles or person-centered treatment plans for persons served Identification of crisis situations for persons served and assist in resolution in these situations Completion of clinical documentation including progress notes and other documentation within established timeframe Completes and submits monthly reports Coordinates/conducts drug screens and alcohol breath tests Monitors persons served progress, safety and medications Monitors and completes discharge planning for persons served Participation in clinical supervision and interdisciplinary treatment team for case consultation Participation in clinical supervision and interdisciplinary treatment team for supervision as outlined in supervision plan Provide consultation within competencies to agency staff Participation in Peer Review and other QI activities Facilitating relationships between ICGH, service user families/legal guardians and various social services and community resources, such as Food and Nutrition Services, housing assistance, healthcare, job training and placement and substance abuse support groups Identification of situations that require resources not available within the agency and completion of appropriate referrals Perform other clinical duties as needed in the program
Qualifications:
Excellent communication and interpersonal skills. Strong recovery oriented skills Ability to work collaboratively in a team environment. Commitment to maintaining confidentiality and ethical practice. Excel at working with persons served to uncover needs of persons served and be able to anticipate these needs. Graduate degree in Social Work or a related field Agreement and adherence to all ethical guidelines for appropriate boards Minimum 2 years experience working with individuals with mental and/or substance abuse and/or behavioral health health issues that includes writing person-centered plans, conducting effective group therapy and providing mental/substance abuse diagnoses General working knowledge of the human services delivery system in North Carolina Valid driver's license, auto insurance and reliable transportation Ability to pass a urine drug screen and background check Meet the physical requirements of performing CPR and therapeutic holds Employee supervisory experience preferred Experience with Electronic Medical Records (EMR) preferred Valid licensure to provide mental/behavioral healthcare in the state of

  • Must be able to operate a variety of automated office machines and other equipment (e.
g. computers, copier, heat sealer, wheel chairs, hand trucks). Must be able to exert up to seventy-five pounds of force occasionally and/or lift, carry, push, pull or otherwise move objects. Physical demands are in excess of sedentary work; position requires walking, standing, reaching, stooping, etc. Will lift no more than forty pounds independently Additional and/or Program Specific Responsibilities•
