Job Description
Our Patient Coordinator is the first point of contact for Cook Community Clinic and sets the standard for services provided at our facility. This individual must exhibit kindness, cultural sensitivity, and compassion for our patients, and show a commitment to the pursuit of health equity among diverse patient populations. The Patient Coordinator is responsible for patient eligibility and enrollment, assisting with scheduling, check-in and check-out, as well as confirming and verifying patient appointments. They also complete appropriate referrals to community resources, as needed. Essential Functions This section describes the primary responsibilities of this position.
