Clinical Intake Supervisor

Medi Home Health and Hospice, a proud member of the Medical Services of America, Inc. family, currently seeks a Full-Time Clinical Intake Supervisor to join our Home Health and Hospice team in West Jefferson (Ashe County), NC . Job Description This person will be responsible for the oversight of the intake processes and management of agency referrals, along with supervising and overseeing the work of Intake Coordinators and Schedulers. Receives and reviews all new patient referrals and relays information to the Scheduler for appropriate assignment. Communicates with referral source and/or physician's office to verify orders Communicates with Sales team and Administrator regarding referrals as appropriate. Reviews and approves requests/orders for services and ensures they are accurate & complete. Receives and reviews weekly schedules for all clinicians providing patient care. Receives, reviews and approves patient admissions, resumptions, re-certifications and/or discharges. Confirms receipt of Face-to-Face encounter documentation or alerts Administrator/DOPS/Sales Team when documentation has not been received. Documents and reports incidents of error and complaints involving scheduling, and/or employee performance to the DOPS or Administrator. Audits physician's orders for accuracy, legibility and content prior to forwarding to physician's office for signature. Collaborates with central coding and clinical staff regarding OASIS. Assists case manager with telephone communications with physician's offices to obtain orders and report clinical facts related to the patient's condition. Directly supervises and oversees the work and performance of the scheduler and intake coordinator. Attends administrative meetings as indicated. Conducts in-home nursing as needed. Completes documentation during the patient visit to ensure patient records reflect accurate and timely information. If unable to complete documentation during patient visit, immediately notifies supervisor regarding reason. Submits within 24 hours. Contributes to team effort by accomplishing related results as needed. Qualifications and Skills RN or LPN degree from an accredited school of nursing; must be licensed to practice in NC Minimum of two years nursing experience (Home Health or Hospice experience strongly preferred).
