Help Drive Quality Care and Exceptional Outcomes As our MDS Coordinator , you'll play a key role in supporting resident care, regulatory compliance, and reimbursement accuracy. Working closely with the interdisciplinary team, you'll help ensure assessments, care plans, and documentation reflect each resident's individual needs while promoting the highest standards of quality care. This position is ideal for an organized, detail-oriented nurse who enjoys collaborating with others and making a meaningful impact on resident outcomes. What You'll Do Coordinate and complete MDS assessments in accordance with state and federal regulations Collaborate with nursing, therapy, dietary, social services, and other departments Assist with developing and updating resident care plans Monitor documentation to ensure accuracy and regulatory compliance Support quality improvement initiatives and clinical outcomes Track assessment schedules and submission deadlines Help maximize reimbursement while maintaining resident-centered care Serve as a resource for team members regarding assessment and care planning processes Promote Ridgewood's mission, vision, and values through excellence in resident care What We're Looking For ✔ Active Registered Nurse (RN) license required ✔ License must be current and in good standing ✔ Current CPR certification required ✔ Strong organizational, analytical, and communication skills ✔ Ability to manage multiple priorities and deadlines ✔ Knowledge of MDS, care planning, and long-term care regulations preferred ✔ High personal integrity and professionalism ✔ Compassionate, resident-focused approach to care ✔ Commitment to quality, accuracy, and continuous improvement Join the Ridgewood Team At Ridgewood Rehab & Care Center, you'll be part of a highly committed team dedicated to providing exceptional care and improving the lives of those we serve.
