Job Description
Portsmouth Regional Hospital is seeking a Clinical Support Coordinator RN to coordinate patient care and clinical workflows in our acute care setting. In this role, you will support physicians and nursing teams with patient assessments, care planning, and discharge coordination, ensuring timely communication and safe transitions of care. You will monitor clinical documentation, assist with patient education, and help optimize unit throughput.
