Medical Coordinator - Developmental Disabilities Services Heart to Heart Home Care We're seeking a compassionate and organized Healthcare Coordinator to join our team at Heart to Heart Home Care. In this role, you'll provide essential healthcare coordination and support services for individuals with developmental disabilities while fostering a nurturing and inclusive environment.
Key Responsibilities:
Healthcare Coordination & Management Coordinate and schedule healthcare services, medical appointments, examinations, and procedures for individuals with developmental disabilities Attend medical appointments with individuals to obtain necessary documentation and information Maintain detailed, accurate medical records and appointment documentation Prepare and manage monthly medical appointment calendars Coordinate with emergency services and healthcare providers as needed Provide assistance with ADL's including hygiene, diapering, meals and other tasks as needed Medication Management Oversee medication administration protocols and staff training Prepare and submit monthly medication orders Conduct regular medication audits to ensure proper supply and compliance Monitor medication administration documentation using Therap system Provide medication education to individuals regarding their prescriptions Program Support & Documentation Assist in developing and updating individualized health plans Prepare and submit reports in compliance with DDD regulations Implement emergency response protocols for medical situations Develop and implement house menus considering dietary needs and preferences Conduct safety orientations and emergency drills for residents Community Integration & Care Create strategies to maximize resident inclusion in community life Transport individuals to community destinations as needed Establish a warm, nurturing environment for residents Maintain interactive relationships that reflect Heart to Heart's values and principles
Required Qualifications:
Experience working with individuals with developmental disabilities Strong organizational and documentation skills Valid driver's license and clean driving record Ability to complete all DDD required trainings Excellent communication and interpersonal skills Demonstrated commitment to person-centered care About Heart to
