Care Finders Total Care LLC, a Modivcare Home care company in NJ is looking for a Per Diem Clinical supervisor. The RN field nursing supervisor is responsible to ensure quality and safe delivery of home health Care services such as supervising LPN's and RN's in a patient's home, evaluate the patient's plan of care and assess their physical and Psychosocial needs.
Schedule:
1 day per week, daytime hours, covering Bergen County and Newark areas. Visits will be scheduled with the patient's nurse present as required.

Compensation:
    Reassessment, and Supervision Visit:
    $65 per visit
      Admission:
      $75 per visit

Performs on-site supervisory and teaching visits to evaluate caregivers Provides clinical orientation to caregivers on assigned home care cases. Participates in the training of new nurses and in-services for field staff. Performs initial nursing assessments Counsels patient/caregiver in meeting nursing and related needs. Assigns and coordinates services provided. Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient's condition/needs. Prepares clinical progress notes ensuring any changes in the patient's condition is reported Administers medication and treatments as prescribed and initiates appropriate preventative and rehabilitative nursing procedures. Monitors clinical records for complete and accurate documentation.
