Job Description

Lead with purpose as the Nurse Director of Care Management at a respected community medical center where strategic oversight drives patient outcomes and operational excellence. ✨ The Nurse Director of Care Management will join a mission-driven hospital recognized for strong quality metrics and a culture of accountability. The Director of Case Management will work in an environment known for high patient satisfaction, low readmission rates, collaborative physician relationships and data-driven performance improvement. This Director of Care Coordination will partner with executive leadership to strengthen care delivery systems. The Director of Utilization Management will play a visible role in advancing patient-centered initiatives. The Nurse Director of Care Management will oversee the case management department within a 43 beds acute care facility. The Director of Utilization Management will guide operations across utilization review, care transitions and discharge planning . This Director of Case Management will ensure regulatory compliance and appropriate reimbursement. The Director of Care Coordination will promote efficient throughput and seamless discharge planning. Reporting to senior nursing leadership, the Nurse Director of Care Management will collaborate with experienced case managers, social workers, physicians and a diverse interdisciplinary team. The Director of Case Management will be responsible for budgeting, staffing strategy, payer relations and quality improvement efforts. They will foster a supportive and professional environment. The Director of Care Coordination will mentor team members and strengthen cross-department communication.
