As a part-time Behavioral Health Care Manager, you will work providing support services to adults with severe mental illness. You'll use your skills, experience and compassion to enhance the lives of this vulnerable population. Jawonio Care Management services are person-centered to meet the comprehensive needs of each individual, to help people become - and remain - healthy while avoiding hospitalization. The Behavioral Health Care Manager will help connect non-Medicaid enrolled individuals to the services they need. Responsibilities include : Identify and assess appropriate participants for care management services including but not limited to, medical, behavioral health, substance abuse-recovery services and social services striving to improve patient's overall quality of life. Engage participants in care management services and related benefits in the community, in-home and as needed, in a court setting. Serve as a referral source connecting individuals to community resources. Educate patients on diagnoses and assist with ensuring continuity of care. Advocate for patient rights. With participant's input, create and maintain Individual Service Plans using person-centered approach. Develop and maintain accurate and timely records. Provide information for participants on vocational, benefits, housing, and employment resources and disseminate information to participants. Provide ongoing support as necessary, specific to needs of each individual participant and their support group. Provide recovery techniques and services in the areas of readiness determination, goals setting, functional and resources assessment; service planning, skill and resource development and discharge-planning.

Transport program participants as needed in agency vehicle.

Education/Experience:

A master's degree in one of the qualifying fields and one year of experience

• or A bachelor's degree in one of the qualifying fields and two years of experience

• A bachelor's degree or higher in ANY field with either: 3 years of experience as a Health Home Care Manager service the SMI or SED population Experience shall consist of providing direct services to people with mental illness Qualifying fields include social work, psychology, nursing, rehabilitation, counseling, community mental health, sociology or other human services field.

Additional Requirements:

A valid NY State Driver's License, ability to drive and driving record acceptable for agency insurance coverage. Must be cleared and maintain acceptable record under the NYS Justice Center mandated criminal background check process. Proficient in use of computers with the ability to navigate multiple software applications, including electronic medical records, excel and word. 17.5 hours per week
