  • Full-time Assisted Living Facility located in Oyster Bay, is currently looking for a Wellness Director/Case Manager to supervise the care given to our residents.
Experience with geriatrics in an assisted living facility or long-term care facility preferred. Position Summary This combined role serves as both Case Manager and Wellness Director for Oyster Bay Senior Campus, coordinating resident assessment and care planning while overseeing the facility's wellness program and medication room operations. The position is responsible for ensuring residents' care and service needs are met and that medication management is conducted safely and accurately in accordance with facility policy. Key Responsibilities Responsibilities include but are not limited to the following: Conducting and documenting admission, periodic, and change-in-condition assessments, including but not limited to the Individualized Service Plan (ISP) and required reassessments. Developing, implementing, and updating individualized service plans in coordination with the resident, family, physician, and interdisciplinary team. Overseeing the medication room, including but not limited to medication storage, security, recordkeeping, and Medication Administration Records (MARs). Supervising and supporting staff responsible for medication assistance and administration and ensuring adherence to facility medication-management procedures. Directing the facility's wellness program and coordinating resident care with internal departments and external providers, including but not limited to home care agencies, hospice, physicians, and pharmacy. Monitoring resident status and ensuring timely follow-up on changes in condition, hospitalizations, returns, and care transitions. Maintaining accurate, complete, and current resident and medication records consistent with facility policy. Serving as a primary liaison among residents, families, staff, and external providers. Qualifications Qualifications include but are not limited to the following: Licensed Practical Nurse (LPN) strongly preferred; medication-management training and experience required. Associate or bachelor's degree in nursing, human services, social work, or a related field, or equivalent relevant experience. Experience in an adult care facility, assisted living, or long-term care setting, including but not limited to wellness, care coordination, or medication oversight, strongly preferred. Demonstrated ability to oversee medication room operations and supervise care staff. Strong assessment, documentation, organizational, and communication skills. Proficiency with electronic records and standard office software.
