Job Description

Fort Hudson Health System is the region s only not-for-profit multilevel post-acute provider of community and facility based care. Our multiple programs and services throughout the region make Fort Hudson an exceptional career opportunity for professionals who value exceptional quality and a passion to serve. The ideal candidate should be experienced in case management/service coordination, inromation, linkages ,and referrals regarding community based services for individuals with disabilities and/or seniors. Job duties include and are not limited to identifying and determining resources in the community, coordinating, and linking clients to providers, actively intervene and advocate on behalf of clients as needed, provide follow up and transitional services, maintain program documentation as required and adhere to appropriate Medicaid billing requirements.
Qualifications:
Qualifying fields of degree: social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing or other human services. Master s Degree in qualifying field or Bachelor s degree with 1 year experience, or Associates degree with 2 years experience. Make Fort Hudson your next and best career move!
