Full job description Behavioral Health Care Manager for Collaborative Care Model (CoCM) Platinum Healthcare Platinum Healthcare is seeking a Behavioral Health Care Manager to join our Collaborative Care team. This role supports patients with common mental health conditions such as depression and anxiety in an assisted living setting.
Key Responsibilities:
Support a caseload of patients and coordinate care with medical providers and psychiatric consultants Screen and assess patients for behavioral health needs Provide education on mental health conditions and treatment options Monitor patient progress and follow up regularly (in person) Deliver brief, evidence-based interventions (e.g., motivational interviewing, behavioral activation) Participate in weekly team case reviews and implement care recommendations Document patient interactions and track outcomes in EHR/registry systems Assist with referrals to mental health, substance use, and social services as needed
Qualifications:
Strong communication and teamwork skills Experience working with patients in a healthcare or behavioral health setting Familiarity with common mental health conditions and basic interventions Ability to engage patients and support treatment plans Comfortable working in person and collaborating with a care team
Pay:
$25.00 - $27.00 per hour
