PATIENT CARE COORDINATOR
Hot Job Main - Fremont, OH 43420 Apply Overview Salary Range $70,000.00 Salary/year Position Type Full Time Job Shift Day Travel Percentage Up to 25% Category Health Care Apply Description Community Health Services is seeking a dedicated Care Coordinator to join our team! This is a full-time position Monday through Friday with no weekends and 10 paid holidays. The Care Coordinator will focus on targeted patient populations to achieve measurable improvements. This position works closely with clinical teams, Quality staff, the EHR Administrator, and community partners to drive evidence-based care, enhance care coordination, and expand remote patient monitoring programs.
Hours for this position are:
Monday 7a-7p, Tuesday- Thursday 8a-5p, & Friday 8a-1p
What We're Looking For:
✅ Registered Nurse (RN) with active Ohio License ✅ Minimum 5 years of experience in a healthcare setting, FQHC experience preferred ✅ Strong understanding of chronic disease management (diabetes, hypertension) ✅ Experience with EHR systems and population health tools (e.g., Azara) ✅ Data analysis, reporting, and performance tracking skills ✅ Ability to manage multiple priorities and meet deadlines
