Job Description

Overview We are seeking a dynamic and experienced Clinical Director to lead our Home Health and Hospice services. In this pivotal role, you will oversee the clinical operations, ensuring the delivery of compassionate, high-quality care that meets regulatory standards and exceeds patient expectations. Your leadership will inspire a team of dedicated healthcare professionals to provide personalized care in diverse settings, including home health, hospice, assisted living, and nursing homes. Join us in making a meaningful difference in patients' lives while fostering a collaborative and innovative clinical environment. Responsibilities Provide strategic leadership and oversight for all clinical operations across home health and hospice programs, ensuring compliance with Medicare, Medicaid, and other regulatory agencies. Supervise nurse management staff and frontline clinicians, promoting best practices in nursing administration, shift management, and care delivery. Develop, review, and update individualized care plans that incorporate residents' physiology, anatomy knowledge, medical terminology, and care needs. Oversee documentation review processes to ensure accuracy and adherence to HIPAA regulations and EMR/EHR systems standards. Monitor clinical performance metrics related to MDS (Minimum Data Set) assessments, documentation quality, and patient outcomes to drive continuous improvement. Lead staff training initiatives on medical records management, medical terminology updates, and compliance with HIPAA privacy rules. Collaborate with interdisciplinary teams to coordinate care plans that address residents' physical health, emotional well-being, and end-of-life needs in hospice care. Ensure proper shift management by scheduling staff effectively to meet patient needs while maintaining regulatory compliance. Maintain comprehensive medical records and ensure all documentation aligns with Medicare guidelines and legal standards. Foster a culture of leadership that emphasizes supervising experience, accountability, compassionate care delivery, and professional growth. Requirements Proven experience in nursing home settings or assisted living environments with a strong background in home health or hospice care. Demonstrated expertise in nursing administration with supervising experience across multidisciplinary teams. Extensive knowledge of acute care practices, medical terminology, physiology, anatomy, and documentation review processes. Familiarity with EMR (Electronic Medical Records) systems and EHR (Electronic Health Records) systems for managing medical records efficiently. Strong understanding of HIPAA regulations to safeguard patient privacy and confidentiality at all times. Experience managing care plans for diverse resident populations with complex medical needs. Knowledge of Medicare regulations related to hospice services and home health reimbursement models. Exceptional leadership skills with the ability to motivate teams, manage shifts effectively, and foster professional development.
