Job Description

We are seeking a dedicated, compassionate, and experienced Certified Advantage Case Manager to join our growing team. This is an excellent opportunity for a motivated professional who is passionate about improving the quality of life for seniors and making a meaningful difference in the community. The ideal candidate will be organized, dependable, and committed to providing high-quality, person-centered care. We are looking for a leader who genuinely enjoys mentoring, developing, and encouraging others and takes pride in helping team members grow professionally. This position includes the opportunity to lead and support a small team of case managers while fostering a positive, collaborative, and supportive work environment. Key Responsibilities Conduct comprehensive client assessments and develop individualized care plans Coordinate, monitor, and evaluate services to ensure clients receive appropriate and timely care Maintain accurate, thorough, and timely documentation in accordance with program requirements and standards Communicate effectively with clients, families, service providers, and interdisciplinary team members Advocate for client needs, preferences, and rights while promoting independence, dignity, and overall well-being Monitor client progress and make appropriate adjustments to care plans and services Lead, mentor, coach, and develop a small team of case managers Provide ongoing guidance, encouragement, and constructive feedback to support team members in reaching their professional potential Foster a positive team culture built on collaboration, accountability, communication, and continuous growth Maintain professionalism, confidentiality, and compliance with all applicable program requirements Qualifications Current Advantage Case Management certification Previous case management or care coordination experience preferred Leadership or supervisory experience preferred Genuine interest in leading, mentoring, and developing a team Strong organizational, time-management, and documentation skills Excellent written and verbal communication skills Strong interpersonal skills and the ability to build positive relationships with clients, families, providers, and team members Ability to work independently, prioritize responsibilities, and manage a flexible schedule Strong problem-solving and critical-thinking skills Commitment to providing compassionate, high-quality, person-centered care to seniors Ability to motivate, support, and hold team members accountable while maintaining a positive and encouraging work environment What We Offer Flexible work structure Supportive and collaborative team environment Meaningful and rewarding work serving seniors in our community Opportunities for professional growth and leadership Competitive pay based on experience and qualifications Benefits available to full-time employees
