Case Manager
- BIP•BHS•Hospital•Tulsa Muscogee Creek Nation•3.
MINIMUM QUALIFICATIONS
Education- A Bachelor's Degree is preferred. Experience
- A minimum of one (1) year relevant work experience Licenses & Certification Case Manager Certification or eligibility for certification is required.
Knowledge & Skills:
Knowledge social services programs and available community resources. Ability to courteously and effectively communicate with participants, CVS, Domestic Violence Probation, AG's office, Department of Health, outside agencies and the general public both orally and in writing Skill in interpersonal and human relations, including teaming and collaboration abilities. Skill in crisis intervention. Ability to work as a team member with other staff to achieve the goals and objectives of the program. Ability to follow guidelines and established Muscogee (Creek) Nation Department of Health (MCNDH), Center for Victim Services (CVS), and MCN Tribal policies and Procedures. Ability to utilize a personal computer and its applications in a Windows environment. Ability to exercise sound independent judgment establishing work priorities. Ability to meet assignment deadline and interpret established policies and procedures. Ability to maintain strict consumer confidentiality at all times. Ability to maintain a confident, professional demeanor. Basic understanding of the Batters Intervention Program. Ability to work in an interdisciplinary setting. Ability to assess and determine a patient's needs. Willingness to learn and navigate the system, being a main point of contact for shared patients and families with the ability to confidently answer questions.JOB SUMMARY
The purpose of the BIP program will be to stop violence, prevent the reoccurrence of violence, ensure accountability for those that do harm, and increase safety for the victims/survivors, the community and public at large. The BIP Case Manager/Support Staff will be responsible for scheduling, assisting, documenting, and reporting on those partisans who are referred to the BIP program. Case Manager/Support Staff will be assigned to a BH location but also may be asked to work at other locations at times.WORK ENVIRONMENT
Work is performed in an office setting and on occasion at arranged job sites as required. Occasional travel will be required in the performance of duties.PHYSICAL DEMANDS
Position requires the demands of normal office work and possibly driving a GSA vehicle to arranged job sites as required.ESSENTIAL FUNCTIONS
Satisfactory job performance will be determined by successful execution of the following: Working collaboratively with CVS, Domestic Violence Probation, AG's office, regarding participants referred to the BIP program. Knowledge of tribal beliefs and customs necessary to courteously and effectively communicate with clients and their families, BHS staff, outside agencies, and the general public both orally and in writing. Work collaboratively with MCN agencies and community partners to assist participants with referral resources and needs to be successful in the course and meet any additional SDOH needs. Develop various community relationships and knowledge of community resources necessary to assist families with identified needs. Ensure confidentiality and participant protection. Work collaboratively with BIP Coordinator/Clinician, MCN agencies and community partners to assist participants with referral resources and needs to be successful in the course and meet any additional SDOH needs. Regular attendance is required to perform the essential functions of this position. Be able to work in an interdisciplinary setting. Complete required interagency and other necessary paperwork.
