This position is responsible for helping new patients and families begin the process of receiving home health services. You will communicate with families, healthcare providers, insurance companies, and internal teams to collect information, process referrals, identify service needs, and make sure each case moves efficiently through the intake process. The ideal candidate is someone who enjoys helping people, communicating by phone, staying organized, following up, and solving problems. You should be comfortable handling multiple referrals at the same time and making sure important details and documentation are not missed. Fluent English and Spanish are required because you will regularly communicate with Spanish-speaking patients, families, healthcare providers, and community members. What You'll Do Receive and process new patient referrals for pediatric home health services. Communicate with patients and families to gather demographic, medical, insurance, and service-related information. Explain the intake process and help families understand the next steps for obtaining home health services. Review referrals for completeness and identify missing or incorrect information. Contact families to obtain required information, documents, and clarification. Communicate with PCPs, physician offices, hospitals, case managers, insurance companies, and other healthcare providers regarding referrals. Obtain physician orders, medical records, Plans of Care, and other documentation required to begin the intake process. Verify insurance information and coordinate with the appropriate internal team regarding eligibility and service requirements. Enter and maintain accurate patient information in company systems and trackers. Coordinate with the Clinical, Prior Authorization, Scheduling, and Outreach teams to move referrals forward. Identify potential barriers that may delay or prevent a patient from receiving services and escalate them appropriately. Follow up consistently with families, providers, insurance companies, and internal departments. Help determine appropriate next steps based on the patient's needs and available services. Communicate professionally and compassionately with families who may be experiencing stressful or challenging circumstances. Maintain accurate intake notes, referral records, follow-up documentation, and other required records. Protect patient confidentiality and follow HIPAA and company privacy requirements. Assist with referral follow-up and community outreach activities as needed. Perform other duties related to patient intake and office operations as assigned. Travel may be required within approximately a 90-mile radius of Bethlehem, PA, to conduct in-person patient intakes and related activities. Mileage for approved work-related travel will be reimbursed. Assist with recruiting and onboarding HHAs for assigned patients, including coordinating required background clearances, collecting HR and employment documentation, and ensuring all required paperwork is completed before the aide begins services. Coordinate with the HR/Compliance team to ensure HHA clearances, credentials, and required onboarding documents are completed and maintained appropriately. What We're Looking For Associate's degree in Healthcare Administration, Human Services, Social Work, Psychology, Business Administration, or a related field preferred. Previous experience in healthcare, patient intake, medical office administration, customer service, care coordination, case management, or a related field preferred. Fluent in English and Spanish — required. Strong verbal and written communication skills in English and Spanish. Compassionate and comfortable communicating with children, parents, caregivers, and families. Comfortable making frequent professional phone calls. Strong organizational and follow-up skills. Excellent attention to detail and ability to accurately enter patient information. Able to manage multiple referrals and prioritize tasks effectively. Strong problem-solving skills and ability to identify missing information or barriers. Comfortable communicating with physician offices, insurance companies, families, and healthcare professionals. Able to handle sensitive or challenging conversations professionally and respectfully. Comfortable using computers, email, Microsoft Office, and learning healthcare software systems. Dependable, proactive, and willing to take ownership of assigned referrals. Able to work independently while collaborating with a team.
