Job Description
Lehigh Valley Health Network (LVHN) seeks an Inpatient Care Manager to coordinate and optimize care for hospitalized patients within our Healthcare & Medical Services department. In this role, you will conduct comprehensive assessments, develop and manage individualized care plans, and collaborate closely with physicians, nurses, and social workers to support safe, timely discharges and high-quality outcomes.
