Pittsburgh, PA Job Details Full-time $37.22 - $56.83 an hour 4 hours ago Qualifications RN License Full Job Description CARE / Pre‑Transplant Nurse Coordinator I ICARE ( Intestinal Care and Rehabilitation ). The program supports pediatric patients with reversible or irreversible intestinal failure. This population includes most children on long‑term total parenteral nutrition (TPN) as well as many children on long‑term enteral feedings (primarily G‑tube or GJ‑tube). This also includes local kiddos that are currently listed for abdominal transplant. The Pre‑Transplant component of the role focuses on managing Status 1 Isolated Small Bowel patients. Some of these patients are followed closely by ICARE, while others are primarily managed by outside teams and are typically seen in person every 6 months to 1 year. Clinic Responsibilities - Thursdays are designated ICARE clinic days (1:00 PM - 4:30 PM).
Clinic responsibilities include:
Reviewing medication lists and current concerns for assigned patients Central line dressing changes and assessments Central line lab draws Ostomy care Assisting with minor procedures as needed Daily Responsibilities Day‑to‑day responsibilities focus on coordination of care for assigned patients, including: Reviewing weekly or biweekly lab results for all TPN‑dependent patients and making necessary adjustments Communicating with home nursing agencies and private‑duty nurses regarding patient status, concerns, and ongoing care Coordinating with home infusion pharmacies and retail pharmacies Placing orders for medications, testing, and durable medical equipment (DME) Completing simple medication prior authorizations Pretty much all things troubleshooting from an outpatient basis. Preparing patients/communicating with school as patients will go to school with central lines, G-Tubes etc. Title and Rate of Pay will be based on Education and Experience Staring Rate of Pay as a Coordinator I is 33.91 2 years recent clinical nursing experience. Demonstrated competence in professional nursing practice. Diploma or Associates Degree in Nursing. BSN preferred. Must obtain a Solid/Strong/Good or higher on annual performance review to be promoted to Clinical Transplant/VAD Coordinator II. It is preferred that within 2 years employee will obtain and maintain certification from the American Board for Transplant Certification (ABTC) as a Certified Clinical Transplant Coordinator (CCTC), CHFN (Certified Heart Failure Nurse), or other UPMC approved certification associated with the service line upon eligibility in order to be promoted to Clinical Transplant Coordinator II.
Licensure, Certifications, and Clearances:
Current Pennsylvania licensure as a Registered Professional Nurse. Certified Clinical Transplant Coordinator OR Certified Clinical Transplant Nurse Registered Nurse (RN) Act 33 / 34 / 73 with renewal Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
