Williamsport, PA Traditional Pediatric & Adult Skilled Home Care (Williamsport) is a leader in the healthcare field, providing exceptional skilled nursing and aide services. We're seeking a Patient Care Coordinator to manage schedules, oversee patient care, and recruit compassionate caregivers. This salaried, weekly-paid role comes with a competitive bonus plan and generous benefits in a supportive, family-owned environment. As the Patient Care Coordinator , you will be responsible for monitoring and supporting the daily activities of services being provided to clients in accordance with state regulations and company policy, while ensuring quality coordination of service for our patients. Duties and responsibilities include, but not limited to the following: Coordinate caregiver schedules and resolve staffing issues. Support patients and families with compassionate communication. Monitor care plans, authorizations, and in-home visits. Ensure all visits are completed and accurately documented for billing, payroll, and EVV compliance Recruit, interview, and onboard reliable caregivers. Build strong relationships with referral sources and grow your patient caseload.
Requirements & Qualifications:
Strong communication and organizational skills. Comfortable using Microsoft Office Experience with scheduling systems/EMRs and PA State Aggregators. Associate's Degree (preferred) At least two (2) years of prior experience in healthcare, home care, or staffing coordination (required) Prior hiring/recruitment experience (required) Why Work at Traditional?
