Job Description

Mental Health organization is excited about hiring an experienced Tailored Care Manager for community-based work. Must possess strong written and verbal communication skills. Have a history of working with underserved communities and be comfortable with mental health and/or physical health populations.
Duties:
Provide case management services to individuals and families in need Conduct assessments to determine client needs and develop individualized service plans based on the unique needs of the consumer.
Care Coordination:
Coordinate and monitor the delivery of services to clients and collaborate with other service providers and professionals, such as social workers and healthcare providers, to provide comprehensive care
Community Engagement:
Assist clients in accessing community resources, support services, and connecting with natural supports and health promotion activities Advocate for consumers' rights and ensure their needs are met Maintain accurate and up-to-date case records
Crisis Management:
providing 24-hour crisis response and support Educating clients on self-managment, self-advocacy, and wellness programs
Requirements:
Bachelor's degree in social work, psychology, or a related field Experience in case management or a related field
Job Type:
Full-time Pay:
$60,000.00 - $62,000.00 per year
