Keen Care Management Inc. is seeking a compassionate, dedicated Clinical Coordinator to join our team! The Clinical Coordinator will work with our affiliated entities to provide ongoing support & maintenance to the Clinical Team. Responsibilities may include (but are not limited to): Follow vendors' requirements to process nurse documentation. Maintain and update patient information in the system. Monitor case status and communicate with associated parties. Send out medical orders via fax, mail, or email to physicians in a timely fashion. Routinely follow up with physicians regarding medical orders Generate and process monthly patient discharges. Conduct non-clinical patient chart reviews. Communicate with vendors to file nurse documentation. Verify RN visit information and enter it in the system. Monitor and report issues to upper management to ensure problems are solved promptly Perform other assigned duties as required.
Requirements :
English-Chinese (Cantonese and/or Mandarin) bilingual preferred At least one (1) year of home care experience preferred. Strong organizational, verbal, written, spoken, and interpersonal skills. Proficiency in basic computer and telephone programs
