How You'll Make a Difference (Essential Functions) The Patient Care Coordinator is responsible for scheduling Homecare and Resource visits. Key Responsibilities Scheduling Complete all Homecare and Resource discipline scheduling (e.g. RN, SW, CNA, etc.). Coordinate staffing to meet patient needs as a result of staff call-ins/absences. Assign patient visits to clinical staff ensuring consistency and following established scheduling best practices. Collaborates with staff and Resource Manager and/or Supervisor regarding assignment changes to increase efficiency. Manages the On Call schedules for the Home Care and Resource staff. May schedule Face-to-face (FFE) visits and telephone calls to patient, family, facility, and caregiver. Provides responsive customer service in a timely manner. Quality and Service Ensure timely assignments of new admissions as well as accurate and timely scheduling of existing patients requiring C.N.A., RN and/or SW support. Report patient and staffing concerns to Resource Manager Problem solve immediate visit scheduling needs. Identify department efficiencies to aide in scheduling. Other Duties Participate in internal work groups, meetings and committees relative to position responsibilities. Ensure compliance with Agrace policies and protocols, state, federal, and other regulations. You Bring (Qualifications) High school diploma or GED or equivalent combination of education and work experience, required Previous scheduling experience, preferred Ability to work weekend rotation.
