Full-Time, Clinic-Based Reports to: Aetos RPM Program Manager Role Summary Aetos Healthcare is looking for an energetic, patient-focused RPM Coordinator to manage Remote Patient Monitoring operations within our Houston partner practices. This is primarily a patient enrollment, engagement and clinic coordination role . The coordinator will work directly with physicians, clinic staff, patients and the Aetos clinical monitoring team to ensure eligible patients are enrolled, connected and actively participating in RPM. Key ResponsibilitiesPatient Identification & Enrollment Work with physicians and clinic staff to identify patients eligible for RPM. Approach eligible patients during clinic visits and through outbound calls. Explain the RPM program clearly and obtain patient consent. Drive enrollment and activation targets for each practice. Maintain an accurate pipeline of eligible, contacted, enrolled and active patients. Patient Onboarding Distribute RPM devices including BP monitors, weighing scales, glucometers and pulse oximeters. Train patients on correct device usage. Confirm successful device activation and transmission. Troubleshoot basic device and connectivity issues. Coordinate replacement devices when required. Patient Engagement Follow up with patients who are not transmitting readings. Encourage regular device usage and adherence. Contact patients who become inactive or disengaged. Serve as the local point of contact for RPM-related patient questions. Clinic Coordination Build strong working relationships with physicians, MAs, nurses and front-office staff. Ensure RPM becomes part of the clinic's routine workflow without creating additional physician workload. Coordinate with the Aetos remote clinical team regarding patient enrollment, monitoring and escalations. Ensure urgent clinical alerts are routed according to established escalation protocols. Documentation & Compliance Maintain accurate documentation of consent, device setup, patient communication and enrollment status. Ensure workflows follow Aetos policies, HIPAA requirements and applicable CMS RPM requirements. Assist with reconciliation of active patients, device status and RPM documentation. Performance Metrics Performance will be measured primarily on: New patients enrolled per month Percentage of eligible patients successfully enrolled Device activation rate Patient adherence and transmission rate Reduction in inactive patients Accuracy of documentation Patient and clinic satisfaction Achievement of practice-level RPM growth targets Qualifications Medical Assistant, LVN/LPN, healthcare coordinator or similar healthcare background preferred. 1-3 years of experience in a physician practice, care coordination, RPM, CCM or patient engagement preferred. Strong patient communication and persuasion skills. Comfortable making frequent outbound patient calls. Comfortable working with healthcare technology and EHR systems. Excellent organization and follow-through. Ability to work independently while coordinating with a remote clinical team. Knowledge of HIPAA and basic healthcare terminology. Bilingual English/Spanish is strongly preferred for the Houston market. Ideal Candidate We are looking for someone who is high-energy, accountable and execution-oriented . This is not a desk-only administrative position. The RPM Coordinator should actively work with clinic staff and patients every day to identify opportunities, enroll patients, solve operational problems and grow the RPM program.
The objective is simple:
more eligible patients enrolled, more patients actively participating, and minimal additional work for the physician.
Pay:
$18.00 - $24.00 per hour
